Client Rights
The rights of every individual served by Mind Dojo — consistent with Pennsylvania law, Joint Commission standards, and federal regulations protecting clients in clinical care.

Mind Dojo is committed to ensuring that every individual we serve is treated with dignity, has full information about their care, and retains decision-making authority over their treatment. The rights below apply to every client at every Mind Dojo location, regardless of program, payment source, or diagnosis.
These rights are consistent with Pennsylvania state law, Joint Commission accreditation standards, DDAP licensing requirements, and — for clients receiving substance use treatment — 42 CFR Part 2.
The Right to Be Informed
- To be informed of your rights at a time when you are able to understand them.
- To receive effective communication tailored to your age, language, and ability.
- To receive accommodations for vision, speech, hearing, or cognitive impairments.
- To request and receive these materials in an alternate format at no cost.
- To have a qualified interpreter provided when needed.
The Right to Be Treated with Dignity
- To be treated with dignity and respect at every point of care.
- To receive services free from discrimination based on race, color, national origin, age, sex, gender identity, sexual orientation, religion, disability, source of payment, or any other protected category.
- To be free from neglect and exploitation, as well as from verbal, mental, physical, and sexual abuse.
- To receive care in a safe environment and to have any concerns about safety addressed promptly.
- To be free from restraint or seclusion used as a means of coercion, discipline, convenience, or retaliation.
Your Authority Over Your Care
- To be involved in making decisions about your care, treatment, or services.
- To give or withhold informed consent before any treatment is provided.
- To refuse care, treatment, or services — and to receive a written explanation of the consequences of refusal.
- To formulate advance directives and have Mind Dojo staff comply with them, consistent with the law.
- To have a surrogate decision-maker involved when you are unable to decide independently.
- To have family, partners, or other support people involved in your care, with your consent.
- To request a second opinion or a change in clinician, where reasonably possible.
Privacy of Health Information
- To have your health information kept confidential in accordance with HIPAA, 42 CFR Part 2, and Pennsylvania law.
- To access, review, and request a copy of your health record.
- To request corrections or amendments to your record.
- To receive a Notice of Privacy Practices describing how your information is used and disclosed.
- To give or withhold informed consent before being recorded, photographed, or included in research.
- To receive detailed disclosure of any research in which you are invited to participate, including its purpose, risks, benefits, and your right to decline without affecting your care.
If Something Goes Wrong
You have the right to raise concerns about any aspect of your care without fear of retaliation. We take complaints seriously and aim to resolve them quickly and fairly.
- To have complaints acknowledged, reviewed, and (when possible) resolved.
- To receive a written response describing the outcome of any formal grievance.
- To file a complaint directly with state and federal regulators, including the Pennsylvania Department of Health, the Department of Drug and Alcohol Programs (DDAP), and the Joint Commission.
- To access protective and advocacy services in the community.
- To continue receiving care during the grievance process, where clinically appropriate.

Additional Protections
Clients receiving care in our medication-assisted treatment (MAT) and substance use programs are granted supplementary protections, including:
- Medication continuity during any grievance review.
- Information about alternative therapy options that may be appropriate for your care.
- Information about community providers and referrals when transitioning out of care.
- Written notice of all decisions related to grievances, including the reason for the decision and the steps to appeal.
- Confidentiality protections under 42 CFR Part 2, which are stricter than standard HIPAA protections for substance use disorder records.

If you believe your rights have been violated and prefer to contact an external agency directly, the following bodies accept complaints about Mind Dojo:

Contacting an outside agency will not affect your care at Mind Dojo.
